Our goal is to make the process as simple and stress-free as possible. If you need assistance at any point, please contact our office rather than your attorney directly so we can respond promptly and keep your matter on track.

COMPLETE YOUR ESTATE PLANNING QUESTIONNAIRE
The questionnaire is the foundation of your estate plan. It provides us with the background information we need to understand your family, assets, and planning goals before we meet.
Please complete the questionnaire as thoroughly as possible. If you’re unsure about an answer, simply make your best estimate—we’ll review everything together during your meeting.
Important: We must receive your completed questionnaire 48 hours prior to your scheduled planning meeting. If it has not been submitted by that time, your appointment will need to be rescheduled so we can make the best use of our time together.
